During the annual Option Period, you have the opportunity to change or enroll in health, dental, vision, and life insurance coverage. If you currently have coverage, you should have received your 2020 Option Period Enrollment/Change Form at your personal address. If you did not and would like to make changes or are not enrolled but would like to be, please complete the Option Period Enrollment/Change Form. Your 2020 Option Period Enrollment/Change Form must be returned to Amber Trammell no later than October 31, 2019.

If you are enrolled in the HealthChoice High or Basic Plan and wish to stay enrolled in that plan, you must complete the online Tobacco-Free Attestation for Plan Year 2020 by November 9, 2019. Members who do not or cannot attest to being tobacco-free will have a higher deductible and out-of-pocket limit for Plan Year 2020.
